A GFR (glomerular filtration rate) score of 78 ml/min/1.73m² measures how well your kidneys are filtering waste, toxins, and extra fluid from your blood. The higher the number, the better your kidneys are working.

A score of 78 falls into what's generally considered mildly reduced kidney function (Stage 2). Normal is typically above 90, so while 78 isn't cause for immediate alarm, it's worth paying attention to — especially with lupus in the picture. For people with lupus, kidney function can be affected by lupus nephritis, where inflammation interferes with how well the kidneys work. Keeping an eye on your GFR over time is really important because it helps track whether things are stable, improving, or declining.

Your doctor may also monitor these alongside your GFR:

- Urine protein test — checks if protein is leaking into urine
- Serum creatinine — elevated levels suggest reduced kidney function
- Blood urea nitrogen (BUN) — measures waste buildup in the blood

The best next step is discussing this result with your rheumatologist or nephrologist (kidney specialist) to understand your personal trend and whether any treatment adjustments are needed.

Katharine, while you need to follow your numbers each time you get a urinalysis, a 78 as the team said is a bit low but not something to worry excessively about-you can still go down quite a ways and still be in stage 2. Eat what is recommended and don’t smoke etc. and don’t worry too much about the 78. Take care of yourself. 💕💖
