I just read some research finding that some of us still receive higher than the recommended daily dose, making us more prone to side effects/toxicity. We should take no more than 5 mg per kg of our body weight daily.
For those less familiar, take your weight in pounds and divide it by 2.2, giving you your weight in kilograms. Then multiply that number by 5. If you are on more than that each day, please talk with your doctors!
The major toxicity is to the retina, which is why we must be sure to have regular retinal scans and visual field testing with our ophthalmologist. With regular monitoring, it's rare to develop this. Even more rarely, heart side effects can also occur.
Ophthalmology studies have determined that 5 mg/kg is the maximum safe dose for the retina for long-term use. DO NOT freak out, please, as I was on it for too high a dose for 5-10 years in the 1990's before this was discovered, and I; 've been on it for 32 years with no toxicity to date.
If you are on 200 mg daily, you must weigh at least 88 lbs. If you are on 400 mg daily, you must weigh at least 176 lbs.
Have a great day, everyone! 🙂
I take 200 mg twice daily, have no vision problems so far. I weigh 180.
@A MyLupusTeam Member, Plaquenil is considered the most important drug for us, as it reduces symptoms, prevents flares, and blood clots, and may even extend our lives. It also doesn't suppress our immunity or make us more prone to infections.
If dosed properly, it's considered the best choice by leading rheumatologists.
I take 200mg two times daily. Never thought about or asked so good to think about
I'm slowly working up to 200 mg alternating with 400 mg a day. I have issues with my rheumatologist as well, too many patients and I can tell she often doesn't have a clue who I am. She's covering all rheumatology diseases, hard to do. Will be switching to one who specializes in lupus but I will have to travel. Don't care. New opthalmologist is very familiar with lupus and it's effects on the eyes. Did tests my other one never did, and is pleased that I have alternating doses of Plaquenil. I lost so much weight, 110 lbs and got foot drop before anyone thought of lupus. I don't have the rash. New doctor too. Has his PhD in pharmacy and in medicine, was an Intensivist and now he's Internal Medicine. Highly recommended by other doctors who feel that he will be better able to manage my other medications in relation to lupus. Hopeful.
