Why is your heart rate dropping and blood pressure low with lupus?

It's concerning that you're experiencing a heart rate in the low 40s along with low blood pressure and fatigue. These symptoms need medical attention soon, as they could be related to several lupus-connected issues.

Low blood pressure can happen with lupus for several reasons:

* Certain lupus medications, especially diuretics or blood pressure medications, can lower blood pressure too much
* Some lupus treatments like rituximab (Rituxan) may contribute to lower blood pressure
* Endocrine disorders that sometimes occur with lupus, such as hypothyroidism or Addison's disease, can cause low blood pressure
* Dehydration or nutritional deficiencies

The combination of low heart rate, low blood pressure, and extreme fatigue you're describing could indicate your heart isn't pumping effectively enough to deliver oxygen to your organs. A MyLupusTeam member shared similar symptoms: "I was light-headed and dizzy with low blood pressure. I went to the ER and came back with a diagnosis of hyponatremia (low sodium levels)." What should you do?

Contact your doctor or rheumatologist right away about these symptoms. A heart rate consistently in the 40s at night, especially with low blood pressure and fatigue, needs evaluation to rule out serious complications.

Keep track of when these symptoms occur and any patterns you notice. This information will help your healthcare provider determine the cause and whether treatment adjustments are needed.
