I keep a notebook and document everything. It takes a lot of time but I have too much going on a and if it isn't written down. I forget to bring it up and ask questions.

I have a document, a chart that keeps important information in case I get too ill to answer questions or when I go to a new doctor. I also have an app - medisafe, that hold some records.

I’ve started keeping a log of symptoms/ dates, along with any questions I have for the doc in my notes on my phone.
