Has anyone seen this and know what it is? Showed pick to rheumatologist, dermatologist and pcp and they all defer to the others with no explanation. I have SLE, Raynauds, Sjogren’s and Crohn’s disease. It started with red puffiness around nail and now I have red, pain gets hot, nail changing shape and color.

What could be causing the red, puffy, painful changes around your nail with heat and nail discoloration?

The symptoms you're describing—red puffiness around the nail, pain, heat, and changes in nail shape and color—could be related to several lupus-associated issues:

Possible causes include:

* Nail fold inflammation from lupus itself, which can cause redness and swelling around the nails
* Raynaud's phenomenon, which you have, can affect blood flow and cause nail changes, discoloration, and issues with the nail bed
* Fungal nail infection, which people with lupus have increased risk for, especially if taking immunosuppressant medications
* Lupus-related blood vessel problems causing redness or discoloration around the nails

A MyLupusTeam member shared similar concerns: "Anyone have red/pink and slightly swollen skin around the nail?"

The heat and pain you're experiencing, combined with the progressive changes, suggest this needs proper evaluation. Since your doctors are deferring to each other, it might help to be more direct about needing a definitive diagnosis and treatment plan.

Consider asking specifically for tests to rule out infection versus autoimmune inflammation, as the treatment approaches would be different.
