Whilst walking slowly on an even surface I felt a sharp pain in my right ankle followed by some moderate pain and discomfort. The next morning a large bruise appeared but I don’t remember twisting my ankle at all. What could be the cause of this?

Yes, our joints and soft tissues can become unpredictably inflamed, especially when we're overdoing it, in my experience. Immersing the affected area in cold/ice water helps a lot, along with rest and elevation. I hope it's better soon!

I just noticed the part of your message about the large bruise. Have you had your blood and platelet counts checked recently, Vicki? Please talk to your rheumatologist about their recommendations and necessary lab tests.

Happens constantly to me. Had a real bad sprain a couple years ago and theses flares are exactly what it feels like but next day they will be gone.

That sounds really uncomfortable and understandably concerning! Lupus can absolutely cause ankle pain and bruising without an obvious injury. Here are some possible lupus-related explanations:

- Joint inflammation (lupus arthritis): Lupus causes inflammation in joints, including the ankle, which can result in sudden pain, swelling, and bruising even without a clear injury
- Tendinitis: Lupus can inflame the tendons around the ankle, causing sharp pain during normal activities like walking
- Osteoporosis/bone fragility: Long-term corticosteroid use and lupus-related bone loss can make bones and surrounding tissues more vulnerable, sometimes causing stress-related injuries
- Peripheral neuropathy: Lupus can damage nerves, which may affect how pain and injury signals are felt, meaning an injury could occur without the usual warning sensations

The bruising appearing overnight is worth paying attention to, as lupus can also affect blood vessels and clotting. It's really important to get this checked out by your rheumatologist or doctor soon, especially given the bruising. They can examine the ankle, rule out a stress fracture, and recommend the right treatment — whether that's rest, anti-inflammatory medications, or further imaging.

Don't try to push through the pain in the meantime!
