Your doctor and ophthalmologist can examine you and help determine the cause. Xanax wouldn't do this.

Narcotic pain meds can cause pinpoint pupils, Amy, but it sounds like you don't take them. I would talk to your doctor about this.

Pinpoint pupils aren't a commonly documented direct symptom of lupus itself, but this is definitely worth taking seriously — especially after 30 years of managing the condition.

A few lupus-related factors could potentially play a role:

- Medications: Long-term use of steroids or other lupus treatments can affect the eyes in various ways
- Nervous system involvement: Lupus can affect nerves, including those that control eye function
- Eye inflammation: Conditions like uveitis, which is linked to lupus, can affect how the eyes respond to light

It's really important to bring this up with both your rheumatologist and an ophthalmologist as soon as possible. Pinpoint pupils can sometimes signal something that needs prompt attention, and given your long history with lupus, your doctors will want to rule out any serious causes.
