related. She told me 40% of patients they don’t know where it comes from. Decided to add a low mg antidepressant for it and try to lower gabapentin eventually. I am frustrated with so many different things going on and how to treat each. I think I will hold off starting the Pamelor because it can cause weight gain and I’m still on 15 mg prednisone for pmr. I am watching diet closely. Actually lost weight but it is muscle. I can’t do much exercise yet. I feel much better but exercise overtaxes my… read more

That sounds like a really exhausting visit — and honestly, "we don't know" is one of the most frustrating things to hear when you're dealing with real, daily symptoms. Your concerns about Pamelor and weight gain are completely valid, especially while on prednisone.

Here are a few things worth considering as you navigate this:

- Talk to your doctor about alternatives — There are other antidepressants used for nerve pain, like duloxetine (Cymbalta), that may have a different side effect profile than Pamelor when it comes to weight
- Muscle loss on prednisone is a known concern — gentle, low-impact movement like stretching or water therapy (when you're ready) can help preserve muscle without overtaxing your body
- Diet focus is smart — prioritizing protein can help protect muscle mass while on prednisone
- Tapering gabapentin should always be done gradually and only under your doctor's guidance to avoid withdrawal effects It's completely reasonable to pause on starting Pamelor until you feel more confident about it. Bringing a written list of your specific concerns — weight gain, muscle loss, current medications — to your next appointment can help your care team find the best path forward for you.

You're clearly being thoughtful and proactive, which truly matters when managing multiple symptoms at once. 💙

If you haven't already tried them, duloxetine, venlafaxine, or pregabalin can also be helpful for painful neuropathy in diabetics and may help you, too, Frankie. You're right--both Pamelor and Ellavil tend to cause weight gain, and the other side effects can be difficult to tolerate.

You may want to try physical therapy to gradually get back into exercising and strengthening.
